Before You Call

Before you call to make an appointment, please review the following list of questions. If you answer YES to any of these questions, please do not make an appointment, as we will not perform any treatments:

• Are you pregnant or breast-feeding?

• Are you taking tetracycline or quinidine?

• Are you under 18 years of age?

Again, if you answer YES to any of the questions above, or are unsure, please do not make an appointment, as under these circumstances, we will not perform a Botox Cosmetic® treatment.

As well, if you have a history of neuromuscular disease, especially if it is currently active, you may not be a good candidate for Botox Cosmetic® therapy. This should be discussed with your treating neurologist or family physician prior to any such treatments being considered, and you should advise us in advance if this is the situation.

If you answer NO to the questions above, please read the Patient Consent Form. You will have an opportunity to discuss this with the physician, and will need to sign it before any treatment. (The physician will bring a copy for your signature.)

To Make Your Appointment

There is a $100 + GST deposit required at the time an appointment is made. If you do not cancel your appointment 48 hours in advance, this deposit will be forfeited. Otherwise, the deposit will be applied against the cost of your treatment.

Please contact our office during business hours at 416 640-6869, or e-mail us at appt1@Botox-at-home.com. Our staff will gladly help arrange an appointment time, manage your appointment deposit, and be able to put you in direct contact with a member of our medical staff.

If you are calling after hours, please leave your name and a telephone number where you can be reached during regular business hours the following day.

If you are calling for a medical emergency, please go to your nearest hospital or clinic.
